Learning Disability Access Navigator

Leicestershire Partnership NHS Trust is recruiting LD Access Practitioners to join the LD Access Team in Leicestershire. You will be the first point of contact for referrals into community learning disability services, completing holistic assessments and managing risk to ensure timely, safe care.

You will work within a supportive multidisciplinary team, benefit from supervision and professional development, and contribute to better outcomes for adults with learning disabilities.
