Care Coordinator/Navigator – Woodgate Valley Health Centre

We are looking for a friendly, organised, and proactive Medical Receptionist / Care Navigator to join our busy and supportive healthcare team.

Please note that this role will be working 20 hours a week over a Thursday to Friday period.

Main duties of the job

This is an important front-line role where you will be the first point of contact for patients, helping ensure they receive the right care from the right service at the right time.

The successful candidate will provide excellent customer service while supporting the smooth day-to-day running of the practice.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.
