Tranquille Care Ltd in Walsall is seeking a Social Care Facilitator to assess adults under the Care Act, develop tailored support plans and coordinate across health, social care and community partners. You will manage your own caseload and conduct reviews with a person-centred approach.
This role requires a valid driving licence and an Enhanced DBS check. Office attendance at the Civic Centre two days per week is expected to support integrated services and safe, coordinated care.
