Reablement Care Coordinator: Independence & Wellbeing

Belmont Recruitment are seeking an experienced Adult Social Care Practitioner to join Sheffield City Council’s Reablement Team on an initial 6‑month temporary contract. This full‑time role is 37 hours per week, Monday to Friday, based in Sheffield.

You will assess the needs of adults requiring social care support, develop person‑centred care and support plans, and complete reviews to promote independence, wellbeing and positive outcomes.
