Job Description:
Wye Valley NHS Trust seeks an experienced Band 6 Occupational Therapist to join the Discharge to Assess (D2A) service in Herefordshire. You will assess patients in their homes post-discharge, plan rehabilitation and arrange equipment to support safe return home. You will work with MDTs to reduce hospital stays and prevent readmissions.
You will triage caseloads, work autonomously in the community, and supervise junior staff while promoting person-centred care and independence at home.
