Medacs Healthcare is seeking an experienced CHC Nurse Assessor (Band 6) to join the Cheshire and Merseyside ICB. The role involves CHC assessments, caseload management, and coordinating care packages, with hybrid working and one office day per week in Merseyside.
You will assess care needs, work with NHS and local authorities, and support NHS-funded Nursing Care and Fast Track pathways in a nurse-led, autonomous role. RGN/RMN/RNLD with CHC experience is essential.
