The Harbour Frailty Service at Newton Abbot Hospital, part of Torbay and South Devon NHS Foundation Trust, seeks a GP to provide senior clinical decision-making and leadership for frailty care, enabling timely assessment and care closer to home.
Responsibilities include frailty coordination, telephone triage, Harbour @ Home virtual ward, SDEC with access to community beds, and contributing to training, development and service improvement across the Harbour team.
