District Nurse & Team Lead – Community Home Care

East Sussex Healthcare NHS Trust is seeking a District Nurse to join our community team. You will deliver high-quality, patient-centred nursing care in patients’ own homes and play a critical role in assessing needs and creating care plans.

The role requires a District Nursing qualification (DN SPQ) and strong communication skills, with the flexibility to work across divisions to ensure continuity of care and oversee a team of Registered Nurses and HCAs.
