Home-Based Health Support Worker – Frailty Care

Nimbuscare Ltd is seeking a dedicated health support role within the Community Frailty Hub in York. You will deliver person-centred care to patients living with frailty in their own homes, helping them remain independent and safe with support from a multidisciplinary team.

The role requires a driving licence and access to your own vehicle for work. Sponsorship is not available; full training will cover safeguarding, infection control, communication and core competencies.
