Home-Based Community Nurse: Palliative & Wound Care

East of England Community Health and Care NHS Trust in Wymondham is seeking a Band 5 Community Nurse on a 6-month fixed term contract to provide in-home care and assessments for patients. You will deliver palliative and wound care, supported by a multidisciplinary team, and contribute to holistic patient planning.

The role requires a registered nurse with NMC registration, a full driving licence, and the ability to work across a 7-day rota, including weekends, to meet patient needs.
