Impactful Health & Employment Coach — One-to-One Support

Vertis Health, Worcester City PCN, is seeking an Employment Coach to work within a multidisciplinary Primary Care Network. You will support individuals with physical, mental health, or social barriers to enter, remain in, or return to employment, conducting assessments and developing personalised action plans.

You will coordinate referrals to specialist services and work closely with healthcare professionals, employers, Jobcentre Plus, and community organisations.
