Home-Based Urgent Care Mental Health Practitioner

Greater Manchester Mental Health (GMMH) Foundation Trust is seeking a post holder to join the urgent care team, delivering high‑quality mental health care in both community and hospital settings. The role emphasises collaborative care planning with service users and carers, following established guidelines and working in a multi‑disciplinary environment.

You will cover night duties on a rotational basis, with occasional weekend work, and may work in other urgent care areas such as A&E liaison.
