Holding asset based person centred conversations to identify strengths, risks and needsCo- producing person centred support plans.Formulating plans to manage risk that support the achievement of desired outcomes.Identifying and intervening in line with Safeguarding Adult procedures when there are allegations of abuse.Taking appropriate action to deal with emergencies, for example, the breakdown of an existing care situation.Undertaking statutory assessments under the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation.
