Hospital Social Worker Level 2 – Strength-Based Care

Warwickshire County Council is seeking a Level 2 Social Worker to join the Hospital Social Care Team. You will manage a caseload, undertake robust assessments, and support care and discharge planning for older adults across Warwickshire’s hospital sites.

You will work five days over seven, including weekends on a rolling rota, with supervision and access to training. A dynamic, strength‑based approach is encouraged in a fast-paced hospital environment.
