Independent Cardiac Rehab Nurse – Community Care

Cornwall Partnership NHS Foundation Trust is seeking a registered adult nurse to provide autonomous, evidence-based care for Cardiac Rehab patients across clinics, homes and community settings. You will promote self-care, work within NICE/BACPR guidelines and collaborate with carers and other professionals to support the patient pathway.

Utilising your clinical skills and communication strengths, you will contribute to secondary prevention goals and help drive uptake of cardiac rehab exercise
