Care & Support Brokerage Specialist

West Sussex County Council is offering an office-based role in the Care and Support Brokerage Team (CPST) to help arrange appropriate care services for people being discharged from hospital and those with social care needs.

We seek someone with strong communication, negotiation, and IT skills (including Excel) who can work with health and social care partners, manage multiple caseloads, and ensure timely, cost-effective support in a busy, collaborative environment.
