Senior Community Nurse: Rapid Response Home Care

Royal Free London NHS Foundation Trust is seeking a Band 6 Rapid Response Nurse based at St Michael’s Primary Care Centre, Enfield, delivering early supported discharge and admission avoidance in patients’ homes within the community.

You will undertake complex, holistic assessments, provide evidence-based care, and coordinate with MDT colleagues to optimise independence and quality of life for patients, carers, and families.
